Mini-Phlebectomy

Ambulatory phlebectomy, also called micro, stab avulsion, or mini phlebectomy is recommended when the size and pressure of blood in a vein is too great for sclerotherapy, and the skin characteristics of the patient make it difficult for sclerotherapy to achieve the desired cosmetic results. The ambulatory phlebectomy procedure bridges the gap between sclerotherapy and more invasive procedures.

Mini-Phlebectomy

Ambulatory phlebectomy is an in-office surgical technique performed under local anesthesia. The procedure involves removing the faulty vein or perforator through minute punctures that require no stitches and leave no scars. A patient that opts for a mini-phlebectomy can walk out of the office and participate in normal daily activities immediately after the procedure. Mini-phlebectomy is often performed in conjunction with Endovenous Laser Ablation (endolaser, EVLT) and other techniques.

What to Expect During Treatment

On the day of the mini-phebectomy, the patient will apply a topical numbing cream given to them by the VTC about two hours ahead of the appointment. The application of the numbing cream at home allows the patient to avoid idly sitting in the doctor’s office for an hour or more, waiting for the anesthetic cream to become fully active. After the two hour numbing period and the patient has arrived at the VTC office, Dr. Al-Misky will do a leg marking. This will provide a ‘map’ on the skin, showing on the body’s surface where the ultrasound visualization has shown where the problem vein and important ancillary veins are under the surface. Dr. Al-Misky then makes an injection of a local anesthetic under the skin, which is almost painless as the numbing cream has desensitized the tissue.

In a sterile setting, the patient is placed on the table. Another dose of local anesthesia, this time near the veins themselves, is injected under color duplex ultrasound guidance. Then the mini-phlebectomy begins as precise stab incisions are made into the veins to be excised. The procedure itself usually takes around a half hour to complete. When the procedure is complete the leg is wrapped with elastic bandages.

Post Treatment

Post instructions are reviewed with the patient, then they walk out of the office and return to their normal daily routine, usually without any pain medication (though about 15% of our patients take Advil or Tylenol if they feel any mild discomfort). Our office staff will usually text you that evening to make sure that you are comfortable and that everything is going according to plan.

You wear the bandages until your Post-Op Visit the next day. Then your leg is unwrapped and examined. You will be slightly bruised; this bruising usually fades away across 2 to 3 weeks. Some people feel tightness in the area of the treated vein indicating that the vein is healing- OTC pain medication can alleviate symptoms. Follow-up sclerotherapy may be needed for any remaining small spider veins.
